SPINAL PROGRAMME QUESTIONNAIRE

Beneficiaries suffering from back and neck pain are encouraged to register on the Scheme’s Spinal programme so that assistance and guidance can be provided on the most appropriate clinical care available within the Scheme’s benefit structure and rules that will achieve the best possible outcome.

Benefits for pain management and/or spinal surgery are subject to registration and adherence to the Spinal programme requirements and are not guaranteed. Beneficiaries must complete the Spinal questionnaire in full and return to the Scheme for evaluation.
